Important Notice for Patients Regarding
the Breach of Protected Health Information

In accordance with 45 CFR Parts 160 and 164 Four Winds is notifying you that it experienced a data breach security incident at its Four Winds Westchester hospital that may have resulted in the breach of personal information about patients treated at Four Winds Saratoga.
