We are seeking applicants for a full time PHP/IOP Patient Coordinator who is responsible for the coordination of admissions and discharges (direct/transfers to the PHP/IOP).  These responsibilities consist of care and case collaboration with patients and/or their families served by the PHP/IOP unit.

Duties include:

  • Being a liaison between program leadership and the patients and families prior to and during their admission.
  • Ensuring intake paperwork and releases of information are completed accurately and in line with agency policy.
  • The Patient Coordinator will also work as a liaison with community providers with a focus on comprehensive discharge planning in compliance with hospital and regulatory agencies.

Qualifications:

  • Associates or Bachelor’s degree in Business/Health Care Administration preferred.
  • Preferred minimum of three years performing patient care management, office functions, preferably in a behavioral health outpatient setting. Proficient typing & Word processing skills required.
  • Must be able to work independently and maintain strict confidentiality.

The schedule is Monday – Friday (8am – 4pm) or (7:30am-3:30pm) with on-call responsibilities. No weekend or holiday requirement.

The hourly pay starts at $18.50/hour with additional compensation for previous work related experience.
