To refer to our Adolescent or Adult Partial Hospitalization Program or Adult Intensive Outpatient Program, please call 518-584-3600 x3001 Monday through Friday from 8:00am to 5:00pm. If you are unsure which of our programs best fits your needs, our admissions care team will assess which of our services is most appropriate for a patient.
Who Can Make a Referral?
Oftentimes patients get referred directly without going through an emergency room. We accept referrals from: family members, emergency rooms, clinicians, schools, physicians, employers, clergy, EAP’s, and managed care organizations. Patients are also welcome to call on their own behalf.
Insurance Coverage
Most insurance plans, including managed Medicaid, pay for Four Winds Hospital services. New York State Medicaid covers inpatient hospitalization for those under the age of 21 and over the age of 65, and partial hospitalization for all adults. Managed Medicaid covers services for all ages.
