To make a referral either to our inpatient or outpatient programs please call 800-546-1754. If you are not sure which of our programs best fits your needs, our admissions care team will assess which of our services is most appropriate for a patient.
Who Can Make a Referral?
Four Winds accepts referrals from: family members, emergency rooms, clinicians, schools, physicians, employers, clergy, EAP’s, and managed care organizations. Patients are also welcome to call on their own behalf.
Insurance Coverage
Most insurance plans, including managed Medicaid, provides coverage for Four Winds Hospital services. New York State Medicaid covers inpatient hospitalization for those under the age of 21 and over the age of 65, and partial hospitalization for all adults. Managed Medicaid covers services for all ages.
